Das großformatige Blatt zeigt eine hellblaue Initiale Q auf einem quadratischen Goldgrund. Im Zentrum steht der Apostel Jakobus Minor mit seinem Attribut, dem Walkerstab. Der Hintergrund besteht aus einem blauen, mit weißen Ranken überzogenen Feld, das sich bis an den linken Blattrand erstreckt. Die Initiale selbst ist mit karminrotem Blattwerk und bunten Ausläufern verziert.
Der Buchstabe Q eröffnet ein Antiphon zu Ehren eines Märtyrers ("Qui me confessus fuerit coram .."). Das ursprüngliche Antiphonar muß daher das Proprium und Commune de sanctis (die den Heiligen gewidmeten Gesänge) umfaßt haben.
Stilistisch vereint die Miniatur lombardische Dekorelemente der Zeit um 1450 (wie die räumlichen Filigranranken und grünen Blüten in den Zwickeln) mit einer neuen, ruhigeren Figurenfassung. Die Abkehr von spätgotischen Extravaganzen verweist auf den Umkreis des Meisters der Chorbücher von San Salvatore in Pavia, weshalb die Entstehung des Werks in Norditalien auf etwa 1480 datiert wird.
